Team Packet

Packet contains 2 rulebooks, information for players and parents, the Touchline publication from SAY to be distributed to the parents, and an electronic team registration form must be completed and submitted to a TCYS Coaching Director, by e-mail, prior to picture night.

Please fill in the registration form completely keeping in mind the following:

• Use the attached sample form to complete yours.  "Team Name" is Tipp City #_____, (your team's number assigned by TCYS).
  

• Please write in the correct jersey number for each player.
  

• All players are considered "new" and should be checked in the "New Player" column.
